Position Title: Social Worker with MSW Preferred
Location: Greenville, OH, US
Employment Status: Full-time position requiring 40 hours weekly
Work Schedule: Monday through Friday from 8:00 am to 4:30 pm, inclusive of on-call shifts during evenings, weekends, and holidays
Responsibilities: Oversight and implementation of psycho-social initiatives tailored to patients and their families in alignment with individualized care plans

• Completes a comprehensive initial and ongoing psychosocial assessment according to accepted standards of practice and agency policy for each patient and family or as requested.
• Assess patients and family members for risk related to abuse/neglect and suicide and those with active or past substance abuse and initiate contracts as deemed appropriate.
• Provide effective counseling approaches and processes for patients and families, including therapeutic and supportive interventions, education, life review, crisis intervention, and suicide prevention when indicated.
• Assist patients and families with end-of-life care planning including knowledge of living wills, DPOA, provision of care for dependents, assessing decision capacity, and funeral planning.
• Develop and implement a plan of care for the patient/family and attend IDG and care conferences as needed.
• Utilize existing community resources to assist patients/families.
• Assist members of the Interdisciplinary Team in understanding the significant social and emotional factors related to the patient’s health care.
• Coordinate visits with other team members to ensure the most effective meeting of patient/family needs.
• Assure continuity of care when a change in location of care is necessary (including admission to a nursing home or respite setting). Facilitate nursing facility placement (PAS/RRs) and arrange transportation.
• Provide education to employees, volunteers, and the community about services available to meet the psychosocial/economic needs of the patient.
• Assist the Bereavement Coordinator as requested.
• Work evenings and weekends as needed to meet the needs of the patients and families.
• Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements.
• Other duties as assigned.
Qualifications:
Education and Experience: • Master of Social Work Degree (may consider a Bachelor of Social Work Degree) from a social work curriculum accredited by the Council on Social Work Education. Master of Social Work preferred.
• Current Social Work licensure in the State of Ohio.
• Minimum of two years of experience in a clinical setting.
• Healthcare and/or Hospice experience preferred.
Special Skills and Requirements: • Shall have a valid driver’s license.
• Excellent customer service skills.
• Detail-oriented with excellent communication, documentation, and organizational skills.
• Ability to follow instructions.
• Ability to work independently without supervision.
• Travel to other sites as needed. Travel may be in inclement weather.
• Ability to lift 25 lbs of supplies or equipment.
• Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking.
