The Health Coach uses a thoughtful and strategic combination of health education, health coaching, and behavioral analysis to help patients overcome barriers to reaching their health goals and to improve their quality of life.This is a remote role. *You must reside in the states of California to be considered a candidate for the role.*
ESSENTIAL JOB FUNCTIONS
Manage patient caseload including preparation, individual coaching visits and charting.
Provide patient coaching, education, and support as well as serve as a primary point-of-contact for patients through various modes of communication.
Evaluate and interpret health needs of patients, provide direct assistance in setting goals, and monitor progress.
Translate scientific language and concepts into understandable, culturally competent information tailored to a patient’s needs.
Consult with primary care provider and care team to provide integrated, high quality patient-centered care.
Support the development and dissemination of health education materials.
Link patients with expert consultants and resources when appropriate.
Collect and analyze data for program evaluation.
Health Coaches with Registered Dietitian certification may provide evaluation and interpretation of dietary health needs of patients as part of overall health needs assessment.
QUALIFICATIONS AND REQUIREMENTS
Bachelors degree and 2+ years of relevant experience in health coaching, health education, health promotion, public health, exercise physiology, nutrition or related field.
Thorough knowledge of health behavior and adult learning theories and their application.
Demonstrated experience in providing nutrition, physical activity, and chronic disease education.
Reside in California and has the ability to have a secure, private location with Wifi internet connection remotely to perform all job responsibilities.
Certification from accredited health coaching program and National Board Certified Health and Wellness Coach (NBC-HWC) strongly preferred.
Background in Behavioral Health coaching strongly preferred.
