Job Overview:

The case manager will offer comprehensive case management and chart management assistance to both clients and staff within the multidisciplinary Goldman Clinic Methadone Maintenance Program, primarily catering to clients funded by medical assistance. This role involves engaging with staff and clients to ensure the delivery of top-notch services while adhering to all clinic licensure and accreditation standards.

Shift Available: 6-2:30P M-F FT

Job Responsibilities:

• Oversee all documentation requests related to Social Security and Department of Welfare applications, ensuring their timely completion and submission.
• Supervise and assist with all documentation requests for transfers in and out of the Goldman Clinic Methadone Maintenance Program.
• Evaluate, create, implement, and track a comprehensive service plan tailored for each client.
• Maintain and regularly update a community resource and referral database that provides supportive services for therapists and staff working with clients, as well as for those referred to case management services, helping them access essential external resources such as housing, job training, healthcare, and transportation services.
• Maintain a solid understanding of public insurance systems, access points, care networks, and the treatment continuum available in the community. This role involves equipping therapists and staff with the necessary knowledge to navigate insurance carriers and gatekeepers, helping clients manage insurance changes and locate providers that accept their plans when needed.
• The ability to secure all necessary IOP authorizations from CBH to ensure that clients referred to the Intensive Outpatient Program have the appropriate insurance coverage. Additionally, it is responsible for obtaining BHSI funding for clients who do not qualify for CBH coverage.
• Oversee the collection and monitoring of clients on a "Fee for Service" basis. This involves assessing clients' treatment needs, making appropriate service referrals, developing tailored treatment plans, and coordinating with other service providers to enhance client care. It may also encompass offering counseling, education, and guidance on various issues related to substance use and treatment.
• Provides support and referrals for clients seeking health, dental, and mental health services, as well as financial assistance, educational opportunities, job training, employment resources, and legal services, among others.
• Verification of attendance for all clients to obtain transportation services.
• Assist clients with housing referrals to shelters or recovery housing.
• Other duties as assigned.
  

Qualifications:

• Associate degree or higher from an accredited college/university and related experience in substance abuse/or mental health.
• Experience in human services field helpful along with knowledge of community resources and dedication to serve others.
• Current First Aid, CPR Certification and bilingual capabilities a plus.
